On 05/23/25 at 8:30am, Licensing Program Analysts (LPAs), Anna Chan and Long Pham conducted an unannounced onsite inspection for the purpose an Annual Random visit. LPAs met with Assistant Director Jennifer Connelly. Assistant Director and LPAs toured the facility inside and outside and the floor and yard plan were verified. Census was taken, the overall census observed was 8 infants and 2 staff in the Infant Room. Infants were doing indoor activities when LPAs arrived. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours ar e 7:00a.m.- 5:30p.m ., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Age-appropriate materials are provided. The facility does not provide food for children. Food and formulas are brought from home. . Formulas were properly labeled and stored. Food prep areas were clean and sanitary. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. Water is provided by Brita water pitcher, and sippy cups are brought by children from home properly labeled with the child’s name on it. Page 1 of 4 Smaller infants aged 12 months and below nap on cribs and bigger infants aged 12 months to 24 months nap on cots. Children brings home sheets and blankets weekly for washing. There is a working carbon monoxide detector and fire extinguisher. The facility conducted an emergency drill on May 9, 2025. Facility met all posting requirement. The California Child Passenger Safety Law was posted by the entrance of the facility. The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material used is wood chips appeared to be enough to absorb falls. There is age-appropriate equipment in the play yard. Drinking water in the outdoor activity space is provided by sippy cups with the child’s name on it. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair. 3 Staff files were reviewed for staff present during the facility inspection on this date. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1596.7995 (a)(1) a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. Proof of immunization against pertussis, measles for the facility were reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporter training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires September 2025. 7 Children's records were reviewed, and there was a separate, complete, and current record for each child. Infant files do have LIC9227 or Individual Infant Sleeping Plan. There is also a complete record of the 15-minute Infant safe sleep log. Sign in/out procedure was reviewed through bright wheel app. In the areas reviewed, the children’s file were in full compliance. Page 2 of 4 Currently, there are medications in the facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. In the areas evaluated, no deficiencies were observed in the California Code of Regulations Title 22, Division 12 at the time of visit. An Inspection and exit interview were completed with Assistant Director Jennifer Connelly. The report was reviewed and discussed. The “Notice of Site Visit” was posted and must be posted for 30 consecutive days. Appeal rights was provided. Page 4 of 4
