Licensing Program Analyst (LPA) Aiddee Nunez conducted an unannounced case management incident inspection in response to a self-report Unusual Incident dated 1/16/2026. During today\342\200\231s visit, LPA met with center director Chamila Gardiwasam. LPA took census in the preschool classrooms and there were 33 preschool age children and 3 staff members. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 1/16/2026, a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported on 1/16/2025 at approximately 10:30am, C1 fell from the climbing play structure and around 12:30pm C1\342\200\231s parent picked up C1 and took C1 to the hospital. It was later determined that C1 had minimal posterior displacement the right elbow. During the investigation, LPA interviewed the director, Staff#1, Staff#2 and tried to interview C1, but C1 did not want to be interviewed by the LPA. Due to additional information needed, the reported incident needs further investigation. Exit interview conducted and report was reviewed with Facility Representative, Chamila Gardiwasam. A notice of site visit was given and must remain posted for 30 days.
