What the inspector wrote

Licensing Program Analysts (LPA) Cynthia Sun conducted an unannounced case management inspection in response to a self-report Unusual Incident dated 04/03/2025. LPA met with Site Supervisor Yessica Hernandez. Census was taken as follows: 4 staff supervising 38 after school children in classrooms #1 and #2. A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 04/03/2025 self reported Unusual Incident Report (UIR): S1 took three children from room 2 to the children\342\200\231s restroom then S1 used the restroom nearby. S1 propped open the door (fully open) and had the children sit on the floor against the wall and wait (the children were not able to see S1 using the bathroom). All three of the children\342\200\231s parents were informed of the incident on the same day by S2 that the incident occurred. S1 resigned on 4/2/2025 and is no longer working with catalyst kids. During today's inspection, LPA inspected facility, observed staff taking children to restroom, interviewed staff and children, obtained facility children, staff rosters, Supervision of Children Handout, CCLD Supervision Video Acknowledgement, and Staff Communication Modules.. Due to insufficient information available at this time, the reported incident needs further investigation. PAGE 1 OUT OF 2 No deficiency was observed during today's inspection. An exit interview was completed. The report was reviewed and discussed. Appeal Rights were discussed. The director was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the licensing office within 15 business days. The director was informed that the 'Notice of Site Visit' must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.00. The 'Notice of Site Visit' must be posted on or adjacent to the door. Page 2 of 2
