What the inspector wrote

On 04/03/25, Licensing Program Analyst (LPA), Anna Chan conducted an unannounced onsite inspection for the purpose of an Annual Random Visit. LPA met with Director Michelle Solorio. LPA and Director toured the facility inside and outside and the floor and yard plan were verified. Census was taken in individual classrooms, a total of 44 preschool children including 9 toddlers and 4 staff. Children were napping when LPA arrived at the facility around 1:00pm. Site Supervisor Perla Zavala-Benitez was present to assist with the visit. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 7:00a.m.- 6:00p.m., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates Staff 1 (S1) did not have the required caregiver background checks or exemptions prior to initial presence at the facility. S1 was asked to leave the classroom and cannot supervise the children without criminal record clearance. Site Supervisor Perla Zavala-Benitez stated, S1 is in an on-boarding training and started 4/1/25. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. AM/PM snacks are provided by the facility, Lunch are brought from home. Only snacks are being prepared in the facility. Food prep area were clean and sanitary. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. Water is provided by the use of Brita water dispenser, and Sports bottles are also brought by children from home with the child’s name on it. Page 1 of 4 Children nap on cots, and bedding is stored individually. Beddings are brought home weekly by children for washing. The facility has a working carbon monoxide detector and fire extinguisher. The facility has conducted an emergency drill on March 20, 2025. Facility met all posting requirement. The California Child Passenger Safety Law was posted by the entrance of the facility. The outdoor activity space was inspected for compliance. The preschool playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material in the preschool playground is sand in the climbing structure such as slide and other climbing equipment which appears to be enough to absorb falls. There is a separate play yard for the Toddler Component and the cushioning material used is artificial grass where the climbing structure is and appeared to be enough to absorb falls. The surface of the outdoor activity space for Toddlers was well maintained and free of hazards. Drinking water in the outdoor activity space is provided by Brita water dispenser and refillable sports bottles with the child’s name on it. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility, and none was observed during today’s visit. 4 Staff files were reviewed for staff present during the facility inspection on this date. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a child care center if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for the facility were reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporter training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 10/2026. Page 2 of 4 5 Children's records were reviewed, and there was a separate, complete and current record for each child. There is a 15-minute sleep log for infants 24 months and under. Sign in/out procedure was reviewed for compliance. Facility uses Brightwheel app for sign in and out. Each authorized representative has their own login. In the areas reviewed the children’s files were found to be in full compliance. Currently there are no medications for children in care. The Director was informed that Licensing Quarterly Updates are available at www.ccld.ca.gov and may request to be added to an email list to receive a Quarterly Update by contacting the Child Care Advocate at childcareadvocatesprogram@dss.ca.gov or at www.ccld.ca.gov LPA provided Guardian Information and website info: https://www.cdss.ca.gov/inforesources/cdss-programs/community-care-licensing/caregiver-background-check/guardian Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. In the areas that were evaluated and based on records reviewed, observation and interview, the facility was not in compliance with the California Code of Regulations, Title 22, Division 12. Health and Safety Code 1596.871(c)(1)(A) Administration of Child Day Care Licensing. A Type A deficiency was cited. See LIC809D. Civil penalty assessed. LPA Chan informed director Michelle Solorio, that this report dated 04/03/25 documents 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA informed Director to provide a copy of this licensing report dated 04/03/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An Inspection and exit interview were completed with Director Michelle Solorio. The report and deficiency were reviewed and discussed. Appeal Rights was provided. The “Notice of Site Visit” was posted on or adjacent to the door. Notice of Site Visit” must be posted for 30 consecutive days. Page 4 of 4
