What the inspector wrote

An informal office meeting was conducted on this date, 3/5/2024, in the Orange County Regional Office. In attendance was Licensing Program Manager (LPM), Patricia Magana, Licensing Program Analyst (LPA), Anna Francesca Chan, Licensee, Junyan Duan and Director, Lilamani Kalugampitiya. The purpose of the informal meeting was to discuss the following citations that were issued on: 07/29/21 and 08/17/23 - 1596.8662(b)(1) Administration for child care licensing Licensee did not ensure 3 out of 10 staff had a current Mandated Reported Training Certificate. Training was completed with staff and certificates were sent to LPA Bootarabi. The director stated they have a checklist in the employee's file. 08/17/23 - 1596.7995(a)(1) Employees or volunteers at day care center; immunization requirements; records; exemptions . Licensee did not ensure 3 out of 10 staff had complete immunization records prior to presence in the facility. The director stated they have a checklist in the employee's file. LPM reminded the facility that immunization is needed prior to working in the facility. 08/17/23 - 101238(a) Buildings and Grounds Licensee did not ensure that the playground is free from hazard and left an unopened box (shed that required assembly) during hours of operation and while children in care are present. Licensee stated that the cabinet was put up and anchored on the wall. 08/17/23 - 101216.3(a) Teacher-Child Ratio Licensee did not ensure proper teacher-child ratio as LPA observed 1 staff supervising 14 children in the playground. As a backup plan, the director stated that they do have some teachers who retired from other facility as substitute. Director stated she would call parents to pick up children if facility is not in ratio. One staff, Ms Annie is the director’s backup in case of the director’s absence. Page 1 of 3 09/22/23 - 101239(o) Fixtures, furniture, equipment and supplies Licensee did not ensure playground equipment was properly anchored to the ground. Licensee stated that the equipment was anchored to the ground. 10/26/23 - 101216.1(c)(1)(A) Teacher Qualification and Duties Licensee did not ensure teacher’s qualifications. LPA observed 24 children with 2 unqualified teachers. The director stated they hired 3 new qualified teachers. Since issuance of the citations, the Director has provided documents and information required pursuant to the plans of correction. Increased unannounced visits to the facility will be conducted by Community Care Licensing for the next 2 years to monitor compliance. The purpose of these increased inspections is to ensure the Department is working with the licensee for continued compliance with applicable regulations and code. The following was discussed with the licensee: 1. The facility must be in compliance at all times. 2. The licensee was advised the facility will be placed on increased required visits for the next two years. 3. The licensee was advised that subsequent citations within the next 12 months may result in civil penalties. 4. The licensee shall view E-learning videos 11A and 11B – Teacher-Child ratios in Child Care Centers found on the CDSS website. A list of E-learning videos and the CDSS website was provided to the licensee. 5. The licensee shall complete the Record Keeping Orientation for Child Care Centers found on the CDSS website at: https://cdss.ca.gov/inforesources/child-care-licensing/how-to-become-licensed/ccc-online-orientations Page 2 of 3 Resources provided: The licensee was provided a list of E-learning videos. Resource Provided: https://startwelloc.org/ In today’s office meeting, TSP was discussed, and licensee agreed to receive TSP services. TSP referral will be sent to licensee. Exit interview conducted with Licensee, Junyan Duan and Director, Lilamani Kalugampitiya, who are in agreement with the above. A copy of this report was provided to Licensee, Junyan Duan and Director, Lilamani Kalugampitiya.
