What the inspector wrote

On 03/28/25, at 1:00pm Licensing Program Analyst (LPA) Chan conducted a Legal/Non-Compliance inspection. LPA met with Director Lilamani Kalugampitiya. Upon arrival the director was informed of the reason for the visit. Census was taken; 6 infant children were being supervised by 2 teachers. Teacher Ana Bojovic continued with the visit as director needed to step away from the facility. An on-site Facility Personnel Report Summary review showed that all facility staff or other individuals who require background checks have received criminal record and child abuse index clearances or exemptions. Children\342\200\231s Records: 3 Infant files were reviewed, and there was a separate, complete, and current record for each child. In the areas reviewed, infant files do have LIC9227 or Individual Infant Sleeping Plan and Infant Needs and Services Plan is up to date. There is also a complete record of the 15-minute Infant safe sleep log of the 3 records reviewed during today\342\200\231s inspection, which is found to be within compliance. Administration of Child Care Licensing: 1 staff file was reviewed. Infant Care Food Service: LPA inspected and observed the feeding bottles and containers for infants were properly labeled. Page 1 of 2 Infant Care Activities: LPA observed 2 infants napping, 4 doing indoor activities. None of the infants were swaddled. Infant Care Center Sleeping Equipment: LPA observed crib sheets were fitted sheets. Staff was reminded about safe sleep. No loose items in the crib, no pillows, blankets or anything on the crib when infants are napping. Infant Care Fixtures, Furniture, Equipment: LPA observed changing table is within arm\342\200\231s reach of a sink. Receipts of Licensing Reports LIC 9224: In compliance At 2:30pm, while LPA was doing an inspection of infant room, LPA observed Child 1 (C1) who is enrolled in the toddler component, being cared in the infant room. Teacher Ana Bojovic stated, C1 just woke up from a nap from Toddler 1 class and was brought to the infant room. In the areas that were evaluated, the facility was in violation of the California Code of Regulations, Title 22, Division 12, Section 101161 (a) Limitations on Capacity . See LIC809D for deficiency cited. Exit interview was conducted and report and deficiency were reviewed and discussed with teacher Ana Bojovic. A notice of site visit was given and must remain posted for 30 days. Page 2 of 2
