What the inspector wrote

On 11/12/25, at 1:45pm Licensing Program Analyst (LPA) Anna Chan conducted a Legal/Non-Compliance inspection. LPA met with Director Lilamani Kalugampitiya. Upon arrival the director was informed of the reason for the visit. Census was taken; 4 infant children were being supervised by 1 teacher. An on-site Facility Personnel Report Summary review showed that all facility staff or other individuals who require background checks have received criminal record and child abuse index clearances or exemptions. Children\342\200\231s Records: 2 Infant files were reviewed, there is also a complete record of the 15-minute Infant safe sleep. Files are in compliance Administration of Child Care Licensing: Director stated there are no new staff for infant license. Therefore, no staff file reviews conducted. I n fant Care Food Service: LPA inspected and observed the feeding bottles and containers for infants were properly labeled. Infant Care Activities : LPA observed 2 infant napping, 2 doing indoor activities. 2 infant file were reviewed. Needs and Services Plan were reviewed and within compliance. None of the infants were swaddled. Infant Care Center Sleeping Equipment: LPA observed crib sheets were fitted sheets. Page 1 of 2 Infant Care Fixtures, Furniture, Equipment : LPA observed changing table is within arm\342\200\231s reach of a sink. Receipts of Licensing Reports LIC 9224: 2 infant files were reviewed and complete with signed acknowledgement of LIC9224 dated 3/5/24. In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit. Exit interview was conducted and the report was reviewed and discussed with Director Lilamani Kalugampitiya. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were Provided. Director stated they are interested to do another (Technical Support Program) TSP for the facility. Page 2 of 2
