Reporting Requirements (f) The items specified in (d)(1)(A) through (H) above shall also be reported to the child's authorized representative.This requirement was not met as evidenced by:Based on staff interviews and record reviews, The facility did not followreporting requiremenst on outbreaks; Total 4 cases of HFM occured prior to reporting to the Department; 1 on 5/27/25, 2 on 6/2/25 and 1 on 6/5/25. However there are no documentation of the facility notifying the children representatives of the outbreak This poses a potential risk to health and safety of children in care.
