What the inspector wrote

On 04/01/2026, an unannounced case management inspection was conducted by Licensing Program Analyst (LPA) V. Trinh, and Licensing Program Analyst (LPA) Madeline Do in response to a self reported incident dated 03/19/2026. LPA met with Facility Representative Leticia Stock and was led on a tour. The overall census was 20 preschool children and 5 staff members. Facility Representative Leticia Stock called Owner/Director Vandana Bhatia as the owner was not in the office. The Owner/Director Bhatia stated that the Director Celida Cambuston resigned on 3/20/2026. LPA reminded the Owner/Director that a new Director Packet must be submitted to Licensing within 30 days. During the inspection, it was determined that the facility is operating within its licensed capacity and in compliance with staff-to-child ratios. Hours of operation are 6:30 AM-6:00 PM, Monday through Friday. Around 4:45pm Owner/Director arrive to the assist the LPA. A review of the Facility Personnel Report Summary conducted on this date 04/01/2026 indicates all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions. The Department received an incident report on 03/19/2026 stating that on 03/18/2026, at approximately 9:00 a.m., while preschool children from Room 3 were being transitioned to the Orchard Garden area, Staff 2 (S2) left Child 1 (C1) at the sandbox in the playground for approximately three (3) minutes. Staff 2 (S2) found C1 unattended in the sandbox area and notified the Owner/Director. During the investigation, LPA conducted interviews with two (2) staff members and attempted to interview three (3) children. LPA obtained pertinent documents for investigative review. Due to additional information needed, the reported incident needs further investigation. Exit interview conducted, and the report was reviewed with Onwer/Director Bhatia. A notice of site visit was given and must remain posted for 30 days. The Director was provided a copy of their appeal rights, and their signature on this form acknowledges receipt of these rights. End of report.
