LPA interviewed 11 staff 2 staff stated they herd (S9) handle C1 in a rough manner. 2 staff stated it was reported to them.Records review showed facility met with employee and Employee Counseling Notice for (S9)was created.

Based on observation, interview, record review, the licensee did not comply with the section cited above. While touring the facility, LPA observed Staff #1 caring for eight children inside classroom #117. LPA observed Staff #1 did not obtain Criminal Record Clearance prior to working in a license facility. LPA observed Staff #1 Determination Status was still in progress which poses an immediate health, safety or personal rights risk to persons in care.

While touring the facility at 9:39 AM, LPA observed a cleaning bottle labeled with Bleach, Epsom Salt Magnesium Sulfate, and Lysol Disinfectant inside an unlocked cabinet directly below the childrens drinking fountain/sink which poses an immediate health, safety or personal rights risk to persons in care.
