In response to a self-reported Unusual Incident Report (UIR) dated 06/01/2026, Licensing Program Analyst (LPA), Deschampe conducted an unannounced case management-incident inspection on this date, 06/03/2026, with arrival at the facility at 12:20PM. Present during today’s inspection was the Facility Representative/Director (FR), Christina Guerrero. Total enrollment is 7, with 5 infants and 2 staff at the time census was taken, due to varied schedules. A review of staff criminal records on this date 06/03/2026 indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearance or exemption and a child abuse index clearance. On 06/01/2026, the UIR was filed with the Orange County Regional Office (OCRO) to self-report an incident from 06/01/2026. The Reporting Party (RP) states that Staff 1 (S1) was taking out trash and opened the back door slightly. As S1 went to shut the door, Child 1 (C1) was sitting near the door. C1 was injured when the door closed. LPA made observations and conducted interviews with 3 staff. Due to additional information needed, the UIR needs further investigation . Exit interview conducted and report was reviewed with the facility representative, Christina Guerrero. A notice of site visit was given and must remain posted for 30 consecutive days. The “Notice of Site Visit” must be posted on, or immediately adjacent to, the interior side of the main door to the facility. Failure to post will result in Civil Penalties of $100.00. End of Report.
