What the inspector wrote

Licensing Program Analysts (LPA) Dean Thompson conducted an on-site inspection for the purpose of a Case Management Incident Inspection on 12/17/2024. LPA met with Director Ashley Neitzke then toured the facility inside and outside. Census was taken, there were 60 preschool-age children with 10 staff. A review of the Facility Personnel Report Summary conducted on today’s date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Today's Case Management inspection is being conducted in response to a self-reported incident, reported to the OC CCL office on 12/13/24, detailing on 12/12/2024, child #1 (C1), having an allergic reaction to eggs and C1 prescribed medication (Zofran) was administered by staff #1 (S1). During today's visit, LPA obtained the Children’s Roster, Care Plan, Medical Records, Health Care Provider Request for Administration of Medication, Non Prescription Medication Administration Consent Form, Non Prescription Medication Administration Record, LIC 627 Consent for Emergency Medical Treatment form, and LIC 701 Physician's Report for C1. Additional information is needed to complete this investigation. No deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit. Exit interview conducted and report was reviewed with Director Ashley Neitzke. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights discussed. Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.
