What the inspector wrote

Licensing Program Analyst (LPA) Giselle Lucero conducted an unannounced case management incident inspection in response to a self-report Unusual Incident dated 03/27/2026. LPA met with Director Nina Betonte. LPA observed a total of 35 preschool children with 4 staff in the classrooms napping. A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On March 30, 2026, a self-reported Unusual Incident Report (UIR) was submitted to the Licensing Office. The facility reported that on Friday, March 27, 2026, Child #1 (C1) was playing in the sandbox area when a log being rolled by another child accidentally rolled over C1’s foot, resulting in swelling and bruising. During today’s visit, LPA conducted staff interviews, collected relevant documentation, and photographed the location where the incident occurred. Due to additional information needed at this time, the reported incident needs further investigation. Exit interview was conducted. The Notice of Site Visit was posted. Director Nina Betonte was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. End of Report.
