What the inspector wrote

Page 1 of 2 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of increasing capacity from 30 school age children to 60 school age children by adding two new rooms (Sister Anna's room and Sister Paula's Room). LPA met with director, Anne Marie H. Dang who toured the facility with LPA. School age children arrived around 3:00 pm. There were 25 school age children with 3 staff. Currently the facility is licensed for 30 school age children in room 5, and 6. The licensee has requested to provide care and supervision for 60 school age children in rooms 5, 6, Sister Anna's room, and Sister Paula's room for ages 5 to 17 years old, Monday through Friday, 2:45 to 5:45 pm. A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Director\342\200\231s office area is located in the front portion of the facility next to the entrance and will serve as the isolation area for ill children temporarily until parents arrive. The staff/adult restroom will be used as the isolation restroom for ill children and is conveniently located to the isolation area. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm Continued on page 2 Page 2 of 2 The indoor activity space measurement is as follows: Room # 5: 1181 square feet Room # 6 155 square feet Sister Anne's Room: 576 square feet Sister Paula's Room : 575 square feet Total indoor space is 2488 divided by 35 = 71.08 (71 children) Total outdoor activity space measurement is as follows: Grass play area 9200 square feet divided by 75 = 112.66 (123 children) Total children toilets and sinks: Toilets: 5 x 15 = 75 children Urinal: 1 x 5 = 5 children Sinks: 5 x 15 = 75 children 2 toilets and 2 sinks for girls bathrooms and 1 toilet, 1 sink, and one urinal in the boys bathrooms are located on the playground. One toilet , one sink for boys bathroom, and on toilet, one sink for girls bathroom are located in the hallway with signs on them for after school use only. Church, parent, and staff restrooms are across from these after school bathrooms. The director was reminded children shall be escorted to and from the restrooms by an adult staff. Orange county Fire authority has approved the requested capacity of 60 school age children. Based on today\342\200\231s measurements, and the sinks and toilets availability, the request of the facility for increasing capacity to 60 children will be final reviewed and discussed with management in the Regional Office. The facility will be notified if any additional information is required. Prior to issue the updated capacity the following are needed. Securing the tall shelves in the rooms to the wall and barricaded the fire place in Sister Paula's room. Please send the proof of corrections no more than 30 days of today's date. Notice of Site Visit must be posted for 30 days or $100 civil penalties will be assessed. An exit interview was completed. The report was reviewed and discussed. Appeal Rights were discussed. The applicant was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Licensing office within 15 business days End of report
