What Happened

Investigation substantiated that staff were regularly left alone with excessive number of children (11-12), violating teacher-child ratio requirements. Staff also left unsupervised when colleagues departed to prepare meals.

Observations

  • Staff member S1 disclosed working alone with 12 children during the day
  • Staff member S2 disclosed usually working with 11-12 children but sometimes alone
  • Staff left unattended when other staff members leave to prepare lunch/snack
  • Staff must walk through empty room to access kitchen
