What the inspector wrote

Page 1 of 2 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of increasing capacity of the toddlers by adding another room (Room # 8 which used to be infant room. Infant room will move to Room # 1). LPA met with director, Jamie Luu who gave a tour of the school to LPA. Currently the facility is licensed for 44 preschool children in rooms # 10 and 16 and 16 toddlers ages 18 months to 36 months old in room # 9 from Toddler Option Component. LPA observed there were 7 staff with 45 children in rooms 10 and 16 including 16 toddlers in room # 9 from Tod dler Option Program. The facility is on the premises of The Liberty Baptist Church and Newport Christian Private School on the same premises. A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The facility has requested to provide care and supervision to 26 toddlers ages 18 months to 36 months from Toddler Option Program in rooms 8 and 9 (room 8 will move to room # 1), Monday to Friday from 7:30 am to 6:00 pm. The facility runs a full time and part time program. Children are dropped off, signed in and out electronically at the entrance of the building to go to assigned rooms. The facility has met all the posting requirements. All the classrooms were measured. The indoor activity space measurement for the current requested capacity is as follows: Room # 8 (Toddler Option Room) is: 406.85 square feet divided by 35=11.62 (12 children) Room # 9 (Toddler Option Room ) is:626.52 square feet divided by 35= 1790 (18 children) Room # 10 (Preschool Room) is: 716.86 square feet divided by 35= 20.48 (21 children) Room # 16 (Preschool Room) is: 814.87 square feet divided by 35= 23.28 (23 children) Total outdoor activity space measurement is as follows: Outdoor space for the preschool playground 3914.93 square feet divided by 75 =52.20 (52 children). Continued on page 2 Page 2 of 2 Total of toilets 6 X 15 = 90 children and there are 3 urinals as well. There is one toilet in the Toddler Option Room # 9 for Toddlers' use and one additional toilet in the restroom is assigned for Toddler Option. Total of sinks: 10 X 15 = 150 children The facility is equipped with carbon monoxide detector. All the classrooms have age appropriate toys and equipment. The director was advised the group of toddlers from Toddler Option Program is maximum 12 toddlers with 2 teachers. The director shall arrange the room so two groups can be accommodated with no more than 12 toddlers in each group. The director was reminded of safe sleep regulations and logging every 15 minutes check on infants and toddlers under 24 months old. A sample of the log was given. The director was reminded that it applies to children from Toddler Option Program who are under 24 months old. Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP . Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the director, Jamie Luu. Based on today’s indoor and outdoor measurements, the sinks and toilets availability, the request of the facility for 28 toddlers in room 8 and 9 and 44 preschool children in room 10 and 16 will be final reviewed and discussed with management in the Regional Office and after receiving the following. The facility will be notified if any additional information is required. Please submit the following no later than 30 days of today's date. 1- Updated waiver request for playground for the increased capacity of 26 toddlers from Toddler Option Component and the total capacity of 70 children and sharing with toddlers. For waiver, please send request letter, LIC 999, LIC 500, outdoor schedule including transitioning time for each group, and a photo of the playground. End of report
