What the inspector wrote

Page 1 of 2 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a follow up investigation regarding the above allegations. This is a follow up for the visit conducted on 9/24/2024. LPA met with director, Jamie Luu and assistant director, Dominique Coccio. Today, there were a total of 58 children with 6 staff including toddlers from Toddler Option Component in the classrooms and on the playground. Rooms 8, 9, 10, 16 were toured. A review of staff records on this date indicated that all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions. The facility is currently licensed for 70 children including 26 toddlers from Toddler Option Component in rooms 8, 9, 10, and 16. The toddler option rooms were Room 8 and 9 and the preschool rooms were 10 and 16. The facility representative had a previous request to increase their total capacity. However, they withdrew their application. A fire clearance obtained on 8/16/2023 when requested change of capacity. The change of capacity application was withdrawn on 6/11/2024. However, the facility is now requesting to add Room 9 which was once licensed as Toddler Option Room to the preschool license. Therefore the facility will operate in Rooms 9, 10, and 16 as preschool rooms and room 8 as toddler option room. However, the number of toddlers are reduced. The facility representative has not requested any change of total capacity of license. they have requested to reduce the number of toddlers in the toddler option program and adding room 9 to the preschool. According to the updated fire clearance received in our office for the changes on 8/16/2023, the restrictions will apply to reflect on the license. Continued on page 2 Page 2 of 2 According to the measurement on file from 4/25/2023 by LPA: The indoor activity space measurement for the capacity is as follows: Room # 8 (Toddler Option Room) is: 406.85 square feet divided by 35=11.62 (12 children) Room # 9 (Preschool ) is:626.52 square feet divided by 35= 1790 (18 children) Room # 10 (Preschool Room) is: 716.86 square feet divided by 35= 20.48 (21 children) Room # 16 (Preschool Room) is: 814.87 square feet divided by 35= 23.28 (23 children) Total outdoor activity space measurement is as follows: Outdoor space for the preschool playground 3914.93 square feet divided by 75 =52.20 (52 children). The facility has a waiver in place to share the playground with toddler option program. The fire clearance approval for maximum occupant load for each room is: Room 8 for up to 10 children ages 18 to 36 months old Room 9 for up to 19 children ages 2 to 6 years old. Room 10 for up to 21 children ages 2 to 6 years old. Room 16 for up to 24 children ages 2 to 6 years old. Total of toilets 6 X 15 = 90 children and there are 3 urinals as well. There is one toilet in the Toddler Option Room between room 8 and room # 9 that the facility has a granted waiver in place on file. Total of sinks: 10 X 15 = 150 children The facility has enough measurement to accommodates 60 children in rooms 9, 10, and 16 and 10 toddlers ages 18 to 36 months in room 8 with the above restricti ons which total capacity will be 70 children. The director Jamie Luu stated she would like to have an updated waiver to share the infant playground with toddler option program in the future. The facility has a current granted waiver to share the preschool playground with toddler option program. The requirement to submit for a waiver was discussed. Notice of Site Visit was posted. The notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100. The director, Jamie Luu and the assistant director, Dominique Coccio were provided a copy of their appeal rights (LIC 9058 1/16) and their signature on this form acknowledges receipt of these rights. In the event additional requirements are needed, the director will be notified . An updated license to reflect the changes will be issued after final review in the office. Exit interview was conducted with director, Jamie Luu and the assistant director, Dominique Coccio.
