Licensing Program Analyst (LPA) Cynthia Sun conducted an unannounced Case Management visit to deliver the Amended Report for a report that was previously completed at the facility on 8/15/25. At 9:20 AM LPA met with Dominique Coccio, who guided LPA on tour of the facility. Census was taken and observed there were 43 children with 8 staff members in the classrooms. A review of the Facility Personnel Report Summary on 09/09/2025 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During visit LPA Sun discussed Amended report with the Director. Director acknowledged and signed Amended report. A copy of the Amended report was provided to the Director. Appeal Rights were explained. Director Dominique Coccio was provided with a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager at the address listed above. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Dominique Coccio. END OF REPORT
