What the inspector wrote

Licensing Program Analyst (LPA) Ms. Ketki Desai conducted an announced on site pre-licensing inspection at the childcare center. It is a Change of Ownership, An Emergency Approval License was issued by the department up to August 9th 2022. LPA met with the Owner/ Applicant Ms. Luu, Jamie along with the facility Manager Mr. Dominique, Coccio who gave a tour of the Child Care Center on the premises of The Liberty Baptist Church. Four rooms and outdoor play yard is designated to the Child Care Center. The applicant has requested to provide care and supervision for 10 Infants in one assigned classroom (Infant room #8 ) , facility has also requested 44 Pre-school age children 3-5 years of age, along with 18, Toddler option (18 months to 36 months) in the assigned three classrooms Monday through Friday 7:30 AM. to 6.00 PM. (Room # 10, 16 (Preschool age ) Room # 9 (Toddler room) . Total capacity of 72 children. LPA observed staff following CDC and Dept of Public Health Guidelines. Due to COVID Pandemic parents shall be encouraged to follow the CDC and COVID requirements. The entrance door is the access door to enter the Preschool premises, parents shall use a pin code to enter the main entrance of the center upon entering the facility through the glass door, there is a front lobby area, where the parent shall sign in the child, facility staff member present shall guide the child to the respective classrooms. A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The facility is on the premises of The Liberty Baptist Church and there is Private school on the same premises. LPA obtained Private School Affidavit certificate. (1) Isolation area is the Director’s office and the sick child shall use the restroom located next to the lobby area there is an additional cots with linens for the sick child. LPA discussed the posting requirements including, but limited to, the following: · Facility License in public area (101160) · Emergency Disaster Plan (LIC 610) · Earthquake Preparedness Check List (LIC 9148) · Parents’ Rights Poster (PUB 393) · Personal Rights (LIC 613A) · Menus / Activity Schedule · Notice of Site Visit (LIC 9213) and Type A deficiencies / Plan of Corrections of Type A deficiencies · Granted Waivers (available for review) · Child Car Seat Law (PUB 269) · COVID Posters. The following were inspected in the indoor activity space in the Infant room · Classroom is adequately equipped with age and size appropriate furniture and equipment · Sign in/Sign out procedure was reviewed and meets regulation requirements (Electronic sign in and out) · There is a working smoke detector, carbon monoxide detector and fire extinguisher that meet statutory requirements · Cubbies available for storage of individual child’s personal belongings and/or bedding · Napping equipment shall be stored in individual cubbies when not in use and Cribs for younger infants and cots for older infants shall be used. · The changing table is within arm’s reach of a sink the changing table is padded with raised sides · There is a designated napping area surrounded by a 4 ft wall with sufficient infant napping equipment's · The infant indoor is physically separate from the preschool component Center policy is that formula/ baby food/ Diapers/ pull ups/ lotions / wipes/ crib linens/ personal utensils/ bowls are to be provided by the parents, every infant shall have their own bin to store personal items. LPA measured the Infant activity room , it is divided into two sections (napping area with cribs and play area) Play area was observed to have age appropriate toys and activities with soft padded cushion. Total indoor activity area Infant room : 216'91 divided by 35 = 6.20 ( 6 infants ) Based on Indoor measurements, facility doesn't have sufficient space for 10 infants. Assigned Infant room can occupy only 6 infants. There is one sink next to the changing station for the Infants 1 sink X 15 = 15 Infants and additional potty chair is available if needed. Infant yard : Outdoor activity area The following were inspected in the outdoor activity space; · Playground is fully enclosed by an appropriate fencing · Outdoor activity space is supplied with age and size appropriate equipment's · An adequate amount of cushioning material consisting of soft cushion covered with turf grass · Adequate shade is provided by canopy and trees Infant yard shall be accessed through the double doors at the end of the hallway and a four seater buggy cart shall be used for infants to transport them to the yard or walks around the hallway . Outdoor measurements Infant yard (Outdoor measurements): 911'63 divided by 75 = 12'16 (12 Infants) Based on Indoor and Outdoor measurements, Facility has sufficient space to accommodate only 6 Infants. Infant yard shall be used on a rotating schedule with the toddlers and a Waiver shall be submitted. Safe Sleep A copy of the 2016 “A Child Care Providers Guild to Safe Sleep” was provided to the applicant/facility representative. Facility has received the Fire clearance approval from Costa Mesa Fire authority for 72 children in the assigned four classrooms ( 44 Preschool / 18 Toddlers and 10 Infants) Based on today's Indoor measurements, facility has the space to accommodate only 6 Infants in the assigned one infant room in classroom # 8 , not 10 infants as requested. Following items were discussed at the time of Exit interview. Criminal Record Statement : Applicant, Ms.Luu, Jamie was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated Safe Sleep : LPA discussed the safe sleep regulations with Applicant Ms. Luu, Jamie and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Applicant Ms.Luu, Jamie of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment . Incidental Medical Services (IMS) APPLICANT KNOWS FACILITY PLANS TO PROVIDE IMS: This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP .
