What the inspector wrote

Page 1 of 2 On 09/30/2025, Licensing Program Analyst (LPA) Deschampe arrived at the facility at 12:00 PM to conduct a case management licensee initiated visit to inspect the facility for the purpose of reactivating the license after roof reconstruction. LPA was met by the Facility Representative/Center Manager (FR), Gerardo Soto. LPA informed the FR the purpose of the visit. FR guided LPA on the inspection of physical plant and grounds. A review of the Facility Personnel Report Summary conducted on this date, 09/30/2025 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA inspected Rooms 17 and 18 and observed all indoor activity space. Upon entering room 17, exposed wires from a light fixture in the ceiling was observed. The electrician had left for the day. Staff storage room in Room 17 was accessible. Room 18 bathroom soap dispenser was not in good condition. Infant's furniture and equipment were safe, age-appropriate, including cribs, mats, tables, chairs, cubbies, bookshelves, climbing equipment, and other activity supplies for infants. Families will provide water, formula, and/or breast milk, with filtered water available from the facility water faucet, if necessary. LPA inspected the children's bathrooms and observed them to be safe, sanitary, and in good repair. LPA inspected the kitchen area and observed the space to be clean and orderly. Page 2 LPA observed the outdoor activity space which includes the playground and patio area. The outdoor equipment and toys were in good repair and free of sharp edges. Animal feces was found on outdoor climbing equipment. The facility will clean and sanitize the area prior to allowing infants access to the space. The outdoor areas were safe and in good repair. There are no bodies of water or weapons on the property. Exit interview conducted and report was reviewed with the facility representative, Gerardo Soto. There is some correction required prior to changing the status to active. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. End of report
