Page 1 On 11/17/2025, Licensing Program Analyst (LPA), Deschampe conducted an on-site inspection for the purpose of an Unannounced Case Management \342\200\223 Licensee Initiated visit for the purpose of a reactivation of the facility license. LPA arrived at the facility at 3:30 PM and was allowed entry by the facility representative, Silvia Magalhaes. LPA explained the purpose of the visit and was guided on a tour of the facility. During the tour of the facility, LPA took the following census: 0 children present as the license is currently inactive. Facility hours are 7:00 AM - 6:00 PM, Monday through Friday. A review of the Facility Personnel Report Summary on this date, 11/17/2025 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code (HSC) section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. During the inspection of the indoor activity space, items which could pose a danger to children (detergents and cleaning compounds) were observed to be stored and out of reach of children. Poisons are stored in a locked storage room. Medications will be centrally stored and inaccessible to children. First-aid supplies were Page 2 complete and stored in each classroom and in the office in a location that is accessible to staff but inaccessible to children. Formula, breast milk, and pureed will be provided and brought from home, per parent/authorized representative preference, and stored properly with personalized labels. Children will nap in a crib meeting the United States Consumer Product Safety Commission standard for each infant who is unable to climb out of a crib. Crib mattresses were firm, covered with vinyl or similar moisture resistant material, and covered with a fitted sheet that is appropriate to the mattress size, fits tightly on the mattress and overlaps the underside of the mattress so it cannot be dislodged. Infant\342\200\231s bedding shall be replaced when wet or soiled and shall be changed daily. Crib mattress covers will be provided by the facility. Crib mattresses shall be wiped daily and maintained in a safe condition with no exposed foam, batting or coils. Cribs did not limit the ability of staff to see the infant. Cribs were free from all loose articles and objects, including blankets and pillows. Cribs did not have any objects hanging above or attached to the side of the crib. Children will nap on cots which shall be maintained in a safe condition, and are properly stored. Children's personal items, including bedding will be clean, stored separately and individually, and in a labeled compartment. The bedding shall be washed weekly, or when soiled or wet. Blankets shall be cleaned or changed when soiled. Napping equipment shall be arranged so that each child has access to a walkway without having to walk over cots of other children. Fixtures, furniture, equipment, and supplies were clean and observed to be in good repair, free of sharp edges, and age appropriate for all children enrolled in care. The children's and staff bathrooms are safe and in sanitary operating condition. The facility has a centralized smoke detection/carbon monoxide system that was tested and cleared by the City of Costa Mesa Fire and Rescue Department Community Risk Reduction Division on 11/12/2025. There are 15 fire extinguishers on site. The facility maintains an indoor temperature within regulation (minimum of 68 degrees and maximum of 85 degrees F or, in extreme heat a maximum of 20 degrees less than the outside temperature). Facility met all waiver, exceptions, and posting requirements , including the California Child Passenger Safety Law, posted by the entrance of the facility. The outdoor activity spaces were inspected for compliance. The playground was enclosed by a fence at Page 3 least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the facility representative shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information This facility will provide Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Commonly Asked Questions about Child Care Centers and the ADA are available at: https://www.ada.gov/resources/child-care-centers/ . Staff files were reviewed, 5 out of 5 staff files were reviewed. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (HSC) 1597.622 states, a person shall not be employed or volunteer at a child care center if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis and measles for staff were reviewed and within compliance. Beginning March 31, 2018, HSC 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications. Page 4 Children are currently not enrolled in the infant program at this time, as the facility is inactive. The facility uses the Hubbe app for parents/authorized representatives to sign in/out with the date/time, complete signature, and personal PIN. LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA reviewed with licensee the following safe sleep best practices: \302\267 Always place infants on their backs for sleeping \302\267 Use only a tight-fitting sheet on the crib or play yard mattress \302\267 Do not hang any items from the crib or above the crib \302\267 Keep all items, including blankets, out of the crib or play yard \302\267 Pacifiers may be used if they do not have items attached to them \302\267 Infants should not be swaddled or have any items covering them while sleeping \302\267 The temperature of the room should be comfortable enough for an adult to wear a t-shirt and not be too hot or too cold. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. The facility representative was informed that Licensing Quarterly Updates are available at www.ccld.ca.gov and may request to be added to an email list to receive a Quarterly Update from www.ccld.ca.gov and select Receive Important Updates link. LPA provided Guardian Information and website info: https://www.cdss.ca.gov/inforesources/cdss-programs/community-care-licensing/caregiver-background-check/guardian Page 5 The facility was in compliance with Title 22 Regulations at the time of inspection. In the event additional requirements are needed, the facility representative will be notified . An active license will be issued once all requirements are met. Exit interview conducted and report was reviewed with the facility representative, Silvia Magalhaes. Appeal Rights were discussed with facility representative. The facility representative was provided a copy of the appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 consecutive days. The \342\200\234Notice of Site Visit\342\200\235 must be posted on, or immediately adjacent to, the interior side of the main door to the facility.
