On March 30, 2026, at 9:00am Licensing Program Analyst (LPA) Long Pham conducted an unannounced case management incident inspection in response to a self-reported Unusual Incident dated 02/02/2026. Upon arrival, LPA met with the director Aysegul Inceoglu and was led on a tour of the facility. LPA observed 4 staff with 36 preschool children. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 02/02/2026, a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported on 01/27/2026 Staff #3 (S3) was called to classroom #2 to check on the child #1 (C1) who had been crying for an extended period of time. Upon entering classroom #2, S3 observed C1 was crying with an ice pack placed on C1’s left wrist. On 01/30/ 2026, S3 reviewed the video playground footage from 01/27/2026 and S3 observed Staff #1 (S1) was pulling C1’s arm aggressively to make C1 walk which caused C1 to injure C1’s left wrist. After investigating, Human Resources placed S1 on paid administrative leave. During today's inspection, LPA conducted a physical plant inspection, interviewed 3 staff, 1 child, reviewed video playground footage, and obtained pertinent documents. S1 stated that S1 did pull C1’s left hand several times to help C1 stand up. S2 admitted that S2 observed C1 resisted and pulled away from S1 on the playground. S3 confirmed that S1 knew C1’s hand was weak and she stills pulled it. Page 1 of 2 During the inspection on 03/30/2026, LPA reviewed the video playground footage on 01/27/2026. LPA observed S1 pulled C1’s left hand with force about 4 times while C1 refused to go back to classroom #2. While S1 was holding C1’s left hand and walked about 15 feet, C1 threw themself on the floor again, and S1 refused to let go of C1’s left hand and aggressively pulled C1 up which caused C1 to injure the left wrist. Based on the information gathered from the self-reported unusual incident report, observations, and interviews with three staff, it was determined that S1 was pulling C1’s left hand aggressively to make C1 stand up and walk which caused C1 to injure C1’s left wrist. The facility was not in compliance and Type A: 101223(a)(3) Personal Rights deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit. (Please see LIC 809D) LPA informed the director Aysegul Inceoglu that this report dated 03/30/2026 document(s) 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. LPA informed the director Aysegul Inceoglu to provide a copy of this licensing report dated 03/30/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the director Aysegul Inceoglu . A notice of site visit was given and must remain posted for 30 days. Appeal Rights were explained. The director was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights.
