Licensing Program Analyst (LPA), Romy Castanon and LPA Silva conducted a Case Management visit to the facility to conduct a prelicesing visit to add a toddler component. LPA met with Applicant Matt Zaky and Executive Director Amber Zaky and toured the indoor and outdoor licensed areas. Applicant is requesting to add Room 1 & 2 as toodler option room. There were 21 children and 3 staff present during today's visit on existing license numbers 304371603. This is a single license conversion and existing license numbers will be consolidated after licensure. Exit interview was conducted and report was reviewed with Applicant Matt Zaky and Executive Director Amber Zaky. End of Report
