Page 1 of 5 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted an announced on site pre-licensing inspection at this childcare center for the purpose of change of ownership from license # 304370353 to operate preschool program. LPA met with the Applicant, Ge Huijun and director, Dishni Kemps. LPA was given a tour with director of the preschool classrooms (2, 3, and 4) childcare center. The applicant has requested to provide care and supervision for 72 children ages 2 to 6 years old, Monday through Friday 6:30 AM. to 6.00 PM. LPA observed staff following CDC and Dept of Public Health Guidelines. Due to COVID Pandemic parents shall be encouraged to follow the CDC and COVID requirements. A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA observed one staff with a total of 10 preschool children. Parents sign in and out manually at the entrance of the building. The entrance is from the rear site of the building entering the playground by opening a gate digitally. There are two locks installed on the gate and a second barrier fence at this entrance for dropping off and picking up children. Upon arrival each child has an health evaluation along with hand washing before they participate in the activities. Isolation area is the Director\342\200\231s office, located at the entrance of the facility and there is an additional cot with bedding for the sick child in the Director's office. The bathroom for sick children is the staff bathroom. Continued on page 2 Page 2 of 5 LPA discussed the posting requirements including, but limited to, the following: \302\267 Facility License in public area (101160) \302\267 Emergency Disaster Plan (LIC 610) \302\267 Earthquake Preparedness Check List (LIC 9148) \302\267 Parents\342\200\231 Rights Poster (PUB 393) \302\267 Personal Rights (LIC 613A) \302\267 Menus / Activity Schedule \302\267 Notice of Site Visit (LIC 9213) and Type A deficiencies / Plan of Corrections of Type A deficiencies \302\267 Granted Waivers (available for review) \302\267 Child Car Seat Law (PUB 269) & \302\267 COVID Posters. Safe sleep posters / SIDS The following was observed in the classrooms # 1, 2, and 3 \302\267 Classrooms are adequately equipped with age and size appropriate furniture and equipment \302\267 Drinking water is available inside through personalized water bottles and drinking fountain. \302\267 Sign in/Sign out procedure was reviewed. \302\267 Facility has in-built smoke detectors & carbon monoxide detectors on the ceiling. There is one fire extinguisher mounted that meets statutory requirements. \302\267 Cubbies available for storage of individual child\342\200\231s personal belongings \302\267 Napping equipment: Children in these program will nap on mats, beddings and blankets will be provided by the parents and washed weekly. Additional sheets are available if needed. \302\267 Facility has a full day and part day program and will provide AM/PM snacks and parents provide lunch to their children. Rooms are well lighted with proper ventilation. Medications are stored in the locked box if any. First Aid kit and Emergency kit is present. Classroom # 2 and 3 have exit doors leading to the parking lot that are equipped with alarm device on them. LPA tested them and they were delayed. The director will order high technology devices. Continued on page 3 Page 3 of 5 LPA measured the classrooms: Classroom # 1 is 436.5 square feet Classroom # 2 is 973.5 square feet Classroom # 3 is 766.12 square feet Total indoor capacity of the 3 classrooms: 2176.12 square feet divided by 35= 62.17 (62 children) Total sinks: 8 X 15= 120 children Total toilets: 7 X 15=105 children Playground measurement: Outdoor activity space is 5082 square feet divided by 75 = 67.76 (68 children) The following was observed: \302\267 Playground is fully enclosed by an appropriate fencing \302\267 Outdoor activity space is supplied with age and size appropriate equipment, outdoor activity toys, \302\267 There are 3 swings on one side of the playground with adequate amount of cushioning (sand). There are climbing structure with slide on the other side of the preschool outdoor space with adequate amount of cushioning (wood chips). Shade is provided by trees and the building shade. Drinking water is provided by children's own water bottles and drinking fountain on the outdoor space. There are 3 gates on the playground. Gate # 1 is for dropping off and picking up children. This gate has two locks on it. Parents enter digitally. There is a 2nd fencing in front of this gate for 2nd barrier. Gate # 2 is always locked with chain and pad lock. Gate # 3 is for entering and exiting staff only. This gate has a latch on the top. An alarm device will be installed on this gate. All these gates lead to the parking lot of the shopping center. The preschool playground can accommodate 68 children. Continued on page 4 Page 4 of 5 . Fire clearance received from Orange County Fire Authority approving the requested capacity of 72 preschool children. The total measurements of the 3 classrooms accommodate 62 preschool children. The director, Dishni Kemps is current on the required Immunization/ Pediatric CPR/ First training and has provided the completion certificate for the Preventive Health training (Nutrition and Lead component) and Mandated Child abuse reporting training. Following items were discussed at the time of Exit interview. Criminal Record Statement : The Director, Dishni Kemps was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Safe Sleep LPA discussed the safe sleep regulations with director, Dishni Kemps and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed the director, Dishni Kemp of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility needs to complete the following items before a final license can be issued: (1) Installing alarm device on the exit door # 3 on the playground leading to parking lot. (2) Permission letters from the two relocation sites in case of disaster situations. (3) Replacing the alarm devices on the exiting doors of rooms of 2 and 3 since they functioned with delay. A license for a total of 62 preschool children ages 2 to 6 years old in rooms # 1, 2, and 3, Monday to Friday 6.30 am to 6:00 PM, will be issued upon receiving the above corrections and Management approval.
