What the inspector wrote

Page 2 LPA conducted a visit on 03/29/2024 and interviewed 4 staff members. During the interview with staff, 2 out of 4 staff disclosed incidents of how the facility commingles children. Staff #2 (S2) recalled two (2) infants being in the preschool classroom for 15 minutes. Staff #3 (S3) recalls incidents of infants and preschool commingling upon opening until other staff arrived. 3 out of 4 staff members stated the facility commingles the children due to a recent issue of low staffing. During secondary visit on 04/10/2024, LPA interviewed 3 children, but no disclosures were made. On 06/04/2024, LPA interviewed 3 out of 5 parents. Parents did not provide information pertinent to the complaint allegation. Based on LPA\342\200\231s interviews, the preponderance of evidence standard has been met, therefore the above allegation staff commingle infants with preschool children is substantiated. California Code of Regulations, Title 22, Division 12, 101161(a) Limitations on Capacity and Ambulatory Status is being cited on the attached LIC9099D. Exit interview was conducted with Office Administrator Binbin \342\200\234Vicky\342\200\235 Zhao. The Notice of Site Visit was posted. Facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100. Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. Licensee shall have LIC9224 (Acknowledgement of Receipt) signed and kept in each child's file. End of Report
