What the inspector wrote

On 12/03/25, at 2:10pm, a case management incident inspection was conducted by Licensing Program Analyst (LPA) Anna Chan who met with Office Admin Binbin Zhao and Director Damarys Chuhuaque and was informed of the purpose of visit. Upon arrival there were 14 preschool children and 2 staff. Children are starting to wake up from nap when LPA arrived. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. A self-reported incident by the facility was received at the regional office on 12/02/25, which stated that on 12/02/25, during drop-off, Staff 1 (S1) witnessed a child (C1) went out of the gate to go to their mother. LPA interviewed S1 who stated they witnessed the incident. The incident happened in the playground during drop-off and transition period around 9am. S1 stated they saw C1 running towards the white gate to follow Mom. S1 stated when they saw C1 climbing the gate, S1 ran toward the gate to follow C1 who was able to go out the first gate. S1 saw Mom just closing the second gate. Mom took C1 home after the incident. Staff 2 (S2) who was at the playground also during the incident stated they heard S1 calling C1's attention and running after C1. Child is not back in school as of 12/3/25. No deficiencies cited during this visit. If any deficiencies are observed, disclosed or discovered, they will be addressed and cited at a later date. Exit interview were completed with director Damarys Chuhuaque. Report was reviewed and discussed.
