What the inspector wrote

An unannounced case management inspection was conducted on 12/29/2025 by Licensing Program Analyst (LPA), Deschampe. LPA arrived at the facility at 9:11 AM in response to a self-reported Unusual Incident Report (UIR) dated 12/09/2025. LPA was allowed entry to the facility by Facility Representative/Administrator (FR), Binbin Zhao. LPA was introduced to Director, Damarys Chihuaque. FR guided LPA on a tour of the facility and observed the following census: 13 children with 4 staff. A review of staff criminal records indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearance or exemption and a child abuse index clearance. On 12/09/2025, the UIR was filed with the Orange County Regional Office (OCRO) to self-report the following alleged incident. FR reported an Authorized Representative (AR) expressed concern about inappropriate behavior from staff with Child 1 (C1). AR told FR C1 touched C1's butthole at pick-up. AR took C1 to the doctor and AR reported the doctor said everything was okay but if AR is concerned, AR should contact Child Protective Services (CPS). FR contacted CPS on 12/09/2025 to report the alleged incident. On this date, LPA made observations, reviewed facility records, and conducted interviews with 6 staff. No children interviews were conducted due to not qualifying for the Ten Step Investigative Interview questions. During staff interviews, 6 out of 6 staff stated they have never witnessed inappropriate behavior between S1 and C1. 6 out of 6 staff stated they have never witnessed inappropriate behavior with any facility staff and C1. No deficiencies cited during this visit. The FR was provided a copy of their appeal rights (LIC9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the OCRO within 15 business days. The "Notice of Site Visit" was posted and must remain posted for 30 consecutive days. Exit interview conducted and report was reviewed with the Facility Representative, Binbin Zhao. End of report.
