What Happened
Complaint investigation substantiated facility operating out of required infant ratio (4:1). Two staff members called off morning of visit, leaving only one fully qualified teacher with 5-7 infant children. Type A repeat violation with civil penalty assessed.
Observations
- At 8:45 AM: 5 infant children with only 1 fully qualified teacher
- At 8:50 AM: 2 more children dropped off still with 1 teacher (7 infants, 1 teacher)
- LPA requested parent stay with child until second staff arrived
- At 9:00 AM: Staff 2 (S2) arrived, bringing ratio to compliant levels
- Two other staff members called off this morning
