Correction check·July 11, 2024

On 7/11/24 at 8:50am, Licensing Program Analyst (LPA), Anna Chan conducted a Plan of Correction (POC) inspection in response to a Type A violation issued on 6/21/24 and 7/09/24,101416.5(b) Staff-Infant Ratio and Type B violation issued on 6/21/24, 101416.3(b) Infant Care Aide Qualification. LPA met with staff Jacqueline Hernandez. LPA went on a walk-through, current census observed were 4 infants and 1 teacher in the playground and 4 infants and 1 teacher in the classroom. Office Administrator Binbin Zhao arrived around 9:45 am. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. The areas cited were re-checked and action plan document was reviewed and found to be corrected. Deficiencies cleared. Clearance letters were provided to the director. No further action needed at this time. POC letters given, and corrections have been received at this time. Office administrator Zhao was reminded that the facility shall notify the Department within 10 days when there is a change of director. And to find a new director or a fully qualified teacher can act as director while facility is finding a permanent director within 30 days. Exit interview conducted and report was reviewed with the Office Administrator Binbin Zhao. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights were explained and provided to facility representative.
