Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted an announced on site pre-licensing inspection at this childcare center for the purpose of change of ownership from license # 304207808 to operate preschool program. LPA met with the director, Perla Zavala Benitez. LPA was given a tour by the director of the preschool of classrooms (Purple Room, Orange Room, Pink Room, Green Room, Teal Room, and Blue Room) of this childcare center.
The applicant has requested to provide care and supervision for 86 children ages 2 to 6 years old, Monday through Friday 7:00 AM. to 6.00 PM.
LPA observed staff following CDC and Dept of Public Health Guidelines. Due to COVID Pandemic parents shall be encouraged to follow the CDC and COVID requirements.
A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA observed 5 staff with a total of 34 preschool children in three classrooms.
Parents sign in and out electronically at the entrance of the building. The entrance is from the front side of the building entering the playground, office, and classrooms. Upon arrival each child has an health evaluation along with hand washing before they participate in the activities.
Isolation area is the Director\342\200\231s office, located at the entrance of the facility and there is an additional cot with bedding for the sick child in the Director's office. The bathroom for sick children is the staff bathroom inside the office area.

Napping equipment: Children in this program will nap on cots, beddings and blankets will be provided by the parents and washed weekly. Additional sheets are available if needed.

Facility has a full day and part day program and will provide AM/PM snacks and parents provide lunch to their children.
Rooms are well lighted with proper ventilation.
Medications are stored in the locked box if any.

LPA measured the classrooms:
Classroom # 1 (Purple Room) is 384.12 square feet
Classroom # 2 (Orange Room) is 436.60 square feet
Classroom # 3 (Pink Room) is 456 square feet
Classroom # 4 (Green Room) is 520 square feet
Classroom #5 (Teal Room) is 675 square feet
Classroom # 6 (Blue Room) is 513 square feet
Total indoor capacity of the 6 classrooms are: 3012.32 square feet divided by 35= 86.07 ( 86 children)
Total sinks: 11 X 15= 165 children
Total toilets: 9 X 15=135 children
Playground measurement: Outdoor activity space is 12406 square feet divided by 75 = 165.41
(165 children).
The following was observed:

There are climbing structure on the front yard section which is equipped with adequate cushioning materials such as wood chips. On the other side of the yard (Back play yard) there are not adequate cushioning materials underneath the climbing structure. .
Shade is provided by trees and, building shade, and awnings.
Drinking water is provided by children's own water bottles.
Fire clearance received from Orange County Fire Authority approving the requested capacity of 86 preschool children. The total measurements of the 6 classrooms accommodate 86 preschool children.

The director, Perla Zavala Benitez is current on the required Immunization/ Pediatric CPR/ First training and has provided the completion certificate for the Preventive Health training (Nutrition and Lead component) and Mandated Child abuse reporting training.
Following items were discussed at the time of Exit interview.
Criminal Record Statement :
The Director, Perla Zavala Benitez was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Safe Sleep
LPA discussed the safe sleep regulations with director, Perla Zavala Benitez and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed the director, Perla Zavala Benitez of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Incidental Medical Services
Incidental Medical Services (IMS) policy was discussed. For IMS information see

. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.

LPA reviewed the LIC 311A, Records to Be Maintained at the Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted.

Facility needs to complete the following items before a final license can be issued:
(1) Tall shelves/cabinets in Purple room, Pink Room, Green Room shall be secured.
(2) Electric outlet for the wall air conditioning in Orange Room shall be covered, the wires were showing.
(3) Adequate of cushioning materials underneath the play structure located on the back play yard.
(4) Removing the play structure from the dirt area (Garden area).
(5) Removing half fencing section on the front yard because the bases are large paint containers and they have sharp edges.
(6) Securing tall plastic cabinets on the outdoor space.
(7) A final picture after cleaning up the outdoor space of back play yard.
A license for a total of 86 preschool children ages 2 to 6 years old in rooms # 1, 2, and 3,4, 5, and 6 Monday to Friday 7:00 am to 6:00 PM, will be issued upon receiving the above corrections and Management approval.
