What the inspector wrote

On 02/28/25, at 12:30pm, Licensing Program Analyst (LPA), Anna Chan conducted an onsite inspection for the purpose of an Annual Random Visit. LPA met with Director Monette Perez and informed of the reason of visit. LPA and Director toured the facility inside and outside and the floor and yard plan were verified. Census was taken in individual classrooms. The overall census observed was 44 Preschool children and 8 staff. Children are preparing to nap when LPA arrived at the facility. The facility is being run by Placentia Yorba Linda School District and the entrance to the facility is located at the parking area of Bryant Ranch Elementary School with a separate entrance from the School. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 7a.m.- 6p.m., Monday through Friday. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Age-appropriate materials are provided. The facility provides AM/PM snacks. Menus are posted in the classrooms where it can be seen by parents. Food is prepared on site. Food is properly stored. Food prep areas were clean and sanitary. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. Drinking water available to children indoors by water jugs and faucet water, and sports bottle are also brought by children from home with the child’s name on it. The children's restrooms were clean and sanitary. Children nap on cots, and bedding is stored individually. Blankets and sheets are brought home for washing weekly. Page 1 of 3 The facility has a working carbon monoxide detector and fire extinguisher. The facility has conducted an emergency drill on February 12, 2025. Facility met all posting requirement. The California Child Passenger Safety Law was posted by the entrance of the facility The outdoor activity space was inspected for compliance. The play ground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material is pour in place rubber surface and appeared to be enough to absorbs falls. Drinking water available to children outdoors by water jugs and faucet water, and sports bottle are also brought by children from home with the child’s name on it. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility and none was observed during today’s visit. 5 Staff files were reviewed for staff present during the facility inspection. Health screening and immunization as required were reviewed. Health and Safety Code 1596.7995 (a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporter training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 08/2025. 5 Children's records were reviewed, and there was a separate, complete and current record for each child. Sign in/out through Eleyo app was reviewed for compliance. In the areas reviewed, the children’s files were found to be in compliance. Page 2 of 3 Currently there are medications at the facility. LPA reviewed storage of medication and equipment/supplies. Based on record review, 1 child’s prescription did not have proper label and has expired on 11/2024. See attached LIC809D. An Inspection and exit interview were completed with Director Monette Perez. The report was reviewed and discussed. Appeal Rights and deficiency were discussed. The director was provided a copy of their appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. The Notice of Site Visit was posted during the visit. The director was informed that the Notice of Site Visit must be posted for 30 consecutive days. Page 3 of 3
