Page 1 of 4 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek and LPA Mila Quinto conducted an announced on site pre-licensing inspection at this childcare center for the purpose of operating a State Preschool Program at this site. LPAs met with the applicant, Coordinator of Child Development Services, from Lowell Joint Unified School District, Ronica Dixon who gave LPAs a tour of the facility. There is one classroom (K1)which has been designated for preschool children ages 2 to 5 years old. The preschool program is located on the ground of Macy Elementary School. All the staff are employees of Unified School District. The applicant has requested to provide care and supervision for 24 preschool children ages 2 to 5 years old in room K1, Monday through Friday from 8:00 am to 3:15 pm. The facility runs a full day program. Parents sign in and out electronically at the entrance of the classroom. Upon arrival each child has an health evaluation along with hand washing before they participate in the activities. Isolation area is the corner side of the classroom for the sick child until the parent picks up the sick child/ren. The adult restroom will be used for this purpose. An additional cot with separate beddings is designated for this area away from the other children. LPA discussed the posting requirements including, but limited to, the following: · Facility License in public area (101160) · Emergency Disaster Plan (LIC 610) Continued on page 2 Page 2 of 4 · Earthquake Preparedness Check List (LIC 9148) · Parents’ Rights Poster (PUB 393) and Personal Rights form · Menus / Activity Schedule · Notice of Site Visit (LIC 9213) and Type A deficiencies / Plan of Corrections of Type A deficiencies · Granted Waivers (available for review) · Child Car Seat Law (PUB 269) The following was observed in one classroom: Age appropriate furniture and equipment’s Toys, tactile equipment's Rooms are well lighted with ventilation. There is sufficient open area for the child to move. Storage cubbies are arranged to store personal belongings Flooring is wood and carpet. Morning and afternoon snacks and lunch are provided by the facility. Water is provided in the children's water containers with their names on them. The facility refills the water for children if needed. Children nap on cots. Parents provide sheets and blankets for children and they are washed on a weekly basis by parent. LPA measured the assigned room for preschool: Room # K1 is: 1233.73 square feet divided by 35 square feet = 35.25 (35 children). Total sinks: 3 X 15= 45 children Total toilets: 3 X 15 = 45 children Total urinals: 1X 15 = 15 children The facility will request a waiver to share the restrooms with TK children of next door. The restrooms are located in an area between State Presch ool and TK classroom. There are one toilet and one urinal in one bathroom and two toilets, and 2 sinks in another bathroom. There is a sink in this classroom. The facility representative was reminded that children shall be escorted to and from the restrooms and be under visual supervision at all times. Continued on page 3 Page 3 of 4 Outdoor activity space is the playground for TK and Kindergarten children of this school site which will be shared with State Preschool children. A waiver request will be sent to our office for this purpose. Outdoor space for preschool children is fully enclosed by an appropriate fencing, Outdoor activity space is supplied with age and size appropriate equipment, and outdoor activity toys. The area is covered with adequate of cushioning. There is a high play structure on the outdoor activity space for preschool children with slide. There are swings on the playground too. There are adequate cushioning (sand) around and under the falling zones. Shade is provided by trees, and 11 umbrellas. Fire clearance received from Orange County Fire Authority approving the requested capacity of 24 preschool children in room K1. The Site Supervisor for this site is Stephanie Perez who is current on the required Immunization/Mandated Child abuse reporting training and Health and Safety Training. Her EMSA approved Pediatric CPR and Pediatric First aid training certificate expires 7/29/2025. Following items were discussed at the time of Exit interview. The facility representative, Roinca Dixon was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. The plan was submitted to LPA during inspection. Continued on page 4 Page 4 of 4 LPA reviewed the LIC 311A, Records to Be Maintained at the Facility, for child’s records, personnel records, administrative records, and documents to be posted. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important license related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication. The facility representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Based on today’s indoor and outdoor measurements, the sinks and toilets availability, the request of the facility for 24 preschool children ages 2 to 5 years old in room K1 will be final reviewed and discussed with management in the Regional Office and after receiving the following corrections. The facility will be notified if any additional information is required. Exit interview conducted and report was reviewed with the applicant, Ronica Dixon. Please send the following to our office no later than 30 days of today's date. 1- Waiver request for sharing playground and restrooms. Please send waiver request letters including the outdoor scheduling, LIC 500, LIC 999, photo of the playground and restroom with posting signs on the stalls and sinks. End of report
