What the inspector wrote

(Page 2) LPA was unable to interview the RP for further information as LPA could not make contact with the RP. During the investigation LPA Lucero interviewed 3 staff members, 3 parents and obtained the facility roster. During staff interviews conducted on 04/29/2024, 3 out of 3 staff interviewed stated the facility has not been out of ratio. LPA attempted to interview 6 parents, however only 3 parents were available for interview. Parents interviewed made no disclosures. Based on LPAs interviews, it has been determined there was insufficient evidence that staff is operating the facility out of ratio. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated. Exit interview conducted and report was reviewed with the Coordinator Ronica Dixon. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were explained. The Facility representative was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. End of Report.
