Page 1 of 4 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted an announced on site pre-licensing inspection at this childcare center for the purpose of operating a preschool program at this site. LPA met with the applicant, School Readiness Coordinator of Early Childhood Education Department of Anaheim Elementary Unified School District, Linda Durand and Site Supervisor, Remedios Herrera. There is one classroom (K5) has been designated for preschool children ages 2 to 5 years old. The preschool program is located on the ground of Anaheim Elementary Unified School District, Price Elementary School. All the staff are employees of Anaheim Elementary Unified School district. The applicant has requested to provide care and supervision for 24 preschool children ages 2 to 5 years old in room K5, Monday through Friday from 7:30 am to 4:00 pm. The facility runs a part time program in two sessions. Morning hours are from 8:00 to 11:00 am and afternoon session from 12:00 noon to 3:00 pm. Parents sign in and out electronically at the entrance of the gate. Upon arrival each child has an health evaluation along with hand washing before they participate in the activities. Isolation area is the Nurse's office, located at the corner of the main school office. A staff person will take the child to the Nurse's office and stays with the sick child until the parent picks up the sick child/ren. There is staff bathroom close to the Nurse's office to be used for sick child/ren temporarily. LPA discussed the posting requirements including, but limited to, the following: \302\267 Facility License in public area (101160) \302\267 Emergency Disaster Plan (LIC 610) Continued on page 2 Page 2 of 4 \302\267 Earthquake Preparedness Check List (LIC 9148) \302\267 Parents\342\200\231 Rights Poster (PUB 393) \302\267 Menus / Activity Schedule \302\267 Notice of Site Visit (LIC 9213) and Type A deficiencies / Plan of Corrections of Type A deficiencies \302\267 Granted Waivers (available for review) \302\267 Child Car Seat Law (PUB 269) & COVID Posters. Safe sleep posters / SIDS The following was observed in one classroom: Age appropriate furniture and equipment\342\200\231s Toys, tactile equipment's Rooms are well lighted with ventilation. There is sufficient open area for the child to move. Storage cubbies are arranged to store personal belongings Flooring is wood. Morning breakfast and afternoon lunch are provided by the facility. Water is provided in the children's water containers with their names on them. The facility refills the water for children if needed. Children do not nap in this program because the program is part time and nap time is not required. LPA measured the assigned room for preschool: Room # K5 is: 782 square feet divided by 35 square feet = 22.34 (22 children). Total sinks: 4 X 15= 60 children Total toilet: 2 X 15 = 30 children The facility will request a waiver to share the restrooms with elementary children. There are 4 toilets, and 3 sinks in the girls' bathroom. The facility needs 2 toilets and 2 sinks to cover the requested capacity for 24 children. The facility representative was reminded that children shall be escorted to and from the restrooms and be under visual supervision at all times. Continued on page 3 Page 3 of 4 Outdoor activity space is the playground for TK and Kindergarten children of this school site which will be shared. A waiver request will be sent to our office. Outdoor space for preschool children is fully enclosed by an appropriate fencing, Outdoor activity space is supplied with age and size appropriate equipment, and outdoor activity toys. The area is covered with adequate of cushioning. There is a high play structure on the outdoor activity space for preschool children with slide. There are swings on the playground too. There are adequate cushioning (foam) around and under the falling zones. Shade is provided by trees and building structure. Fire clearance received from Orange County Fire Authority approving the requested capacity of 24 preschool children in room K5. The Site Supervisor, Remedios Herrera is current on the required Immunization/Mandated Child abuse reporting training. Her EMSA approved Pediatric CPR and Pediatric First aid training certificate expires 3/2025. The site supervisor has Preventative Health Training. However she has been enrolled take this class on 9/6 and 9/7/2023. Following items were discussed at the time of Exit interview. The facility representative, Lynda Durand was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. The plan was submitted to LPA during inspection. Continued on page 4 Page 4 of 4 LPA reviewed the LIC 311A, Records to Be Maintained at the Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important license related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication. The facility representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Based on today\342\200\231s indoor and outdoor measurements, the sinks and toilets availability, the request of the facility for 22 preschool children ages 2 to 5 years old in room K5 will be final reviewed and discussed with management in the Regional Office and after receiving the following corrections. The facility will be notified if any additional information is required. Exit interview conducted and report was reviewed with the applicant, Lynda Durand. The measurement of the room is accommodating 22 children only. 1- Waiver request for sharing playground and restrooms. Please send waiver request letters including the outdoor scheduling, LIC 500, LIC 999, photo of the playground and restroom with posting signs on the stalls and sinks. End of report
