Licensing Program Analyst (LPA) Mila Quinto conducted an announced on site pre-licensing inspection at this childcare center for the purpose of operating a State Preschool Program at this site. LPA met with the applicant, Child Development/School Readiness Coordinator, Susie Estrada. LPA and applicant toured the facility inside and outside and floor and yard plan were verified. There is one classroom (P1) which has been designated for preschool children ages 3 to 5 years old. The preschool program is located on the ground of Glen H. Dysinger Elementary School. All the staff are employees of the Centralia Elementary School District. The applicant has requested to provide care and supervision for a total of 48 preschool children offering 2 sessions with a total capacity of 24 children per session ages 3-5 years old. The operational hours are from Monday -Friday 8am-11am and 11:30am-2:30pm. This facility runs a half day program per session. Parents sign in and out at the entrance of the classroom. Upon arrival each child has an health evaluation along with hand washing before they participate in the activities. Isolation area is the corner side of the classroom for the sick child until the parent picks up the sick child/ren. The adult restroom will be used for this purpose. An additional mat with separate beddings is designated for this area away from the other children. Page 1 of 4 LPA discussed the posting requirements including, but limited to, the following: \302\267 Facility License in public area (101160) \302\267 Emergency Disaster Plan (LIC 610) \302\267 Earthquake Preparedness Check List (LIC 9148) \302\267 Parents\342\200\231 Rights Poster (PUB 393) and Personal Rights form \302\267 Menus / Activity Schedule \302\267 Notice of Site Visit (LIC 9213) and Type A deficiencies / Plan of Corrections of Type A deficiencies \302\267 Granted Waivers (available for review) \302\267 Child Care Seat Law (PUB 269) The following was observed in the classroom: Age appropriate furniture and equipment\342\200\231s Toys, tactile equipment's Rooms are well lighted with ventilation. There is sufficient open area for the child to move. Storage cubbies are arranged to store personal belongings Flooring is carpet only. Breakfast provided for session 1 and lunch provided for session 2. Water is provided in the children's water containers with their names on them. The facility refills the water for children if needed. LPA measured the assigned room for preschool with total measurement of 836.73 divided by 35 square feet = 24 children. Total sinks: 8 X 15= 120 children Total toilets: 8 X 15 = 120children Total urinals: 1X 15 = 15 children The facility will request a waiver to share the restrooms with elementary school restroom adjacent to the preschool building. There are 4 toilets and 3 sinks in one bathroom and 3 toilets, 1 urinal and 3 sinks in another bathroom. There is also 1 toilet and 2 sinks in the classroom. The facility representative was reminded that children shall be escorted to and from the restrooms and be under visual supervision at all times. Page 2 of 4 Outdoor activity space is the playground for Kindergarten children of this school site which will be shared with State Preschool children. A waiver request will be sent to our office for this purpose. Outdoor space for preschool children is fully enclosed by an appropriate fencing, Outdoor activity space is supplied with age and size appropriate equipment, and outdoor activity toys. The area is covered with adequate of cushioning. There is a high play structure on the outdoor activity space for preschool children with slide. There are swings on the playground too. There are adequate cushioning made of rubber turf around and under the falling zones. Shade is provided a shade cover. LPA measurement of the outdoor activity area is 3115.80 square feet divided by 75= 42 preschool children. Fire clearance received from Orange County Fire Authority approving the requested capacity of 48 preschool children in room P1. The Site Supervisor for this site is Denise Bahena who is current on the required Immunization/Mandated Child abuse reporting training and Health and Safety Training. The Pediatric CPR and Pediatric First aid training certificate expires 5/25/2023 however it is not EMSA approved. Following items were discussed at the time of Exit interview. The facility representative, Susie Estrada was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. The plan was submitted to LPA during inspection. Page 3 of 4 LPA reviewed the LIC 311A, Records to Be Maintained at the Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important license related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication. The facility representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Based on today\342\200\231s indoor and outdoor measurements, the sinks and toilets availability, the request of the facility for 48 preschool children ages 3 to 5 years old in room P1 will be final reviewed and discussed with management in the Regional Office and after receiving the following corrections. 1- Waiver request for sharing playground and restrooms. Please send waiver request letters including the outdoor scheduling, LIC 500, LIC 999, photo of the playground and restroom. 2- Please provide an updated EMSA approved Pediatric CPR and Pediatric First aid training certificate for Site Supervisor, Denise Bahena. The facility will be notified if any additional information is required. Exit interview conducted and report was reviewed with the applicant, Susie Estrada.
