What the inspector wrote

Page 2. On 1/15/2025, the Orange County Child Care Office received a complaint alleging: 1) Facility did not meet child’s toileting needs. Reporting Party (RP) stated that child 1(C1) was dropped off at facility at 7AM and picked up at 3:45PM/4:00PM and C1’s diaper was soaked with fecal matter. During the investigation, LPA toured the facility, obtained a copy of Kiddie Academy Personnel Roster, Sign in/out Sheets, Diaper Changing Logs, Kiddie Academy Diapering and Independent Toileting Training: Policy and Procedure, Reviewed Tadpoles App for documented Diaper Changes, and conducted interviews with 10 staff and 5 parent interview. No children interview was conducted due to children being nonverbal or have limited vocabulary. During visit dated 1/21/2025 , LPA observed classrooms diaper changing areas and procedure. All 10 staff (S1-S10) interviewed stated that Kiddie Academy has a staff diaper policy and procedure that they must follow when changing diapers. All ten (10) staff interviewed stated that a diaper should be checked every 2 hours and that they must document the time changed and whether it is wet, dry, or a bowel movement (BM) into the app called Tadpoles. During LPAs’ records review of documented diaper changes in Tadpoles and sign in and out sheets, on 1/14/2025 C1 was in care for at least 7.5 hours with being dropped off at 8:12AM and picked up at 4:00PM. During C1s time at facility there was one (1) documented diaper change in Tadpoles at 11:00AM. During parent interviews. LPA called 8 parents and LPA was able to interview 5 parents. The 5 interviewed parent did not divulge any information pertaining to the allegation. Based on LPA observations, interviews, and records review, the preponderance of evidence has been met; therefore, the allegation s of, facility did not meet child's toileting needs is substantiated. California Code of Regulations, Title 22, Division 12, Chapter 1 Section 101428(b)(2) Infant Care Personal Services was being cited on the LIC9099D . See LIC9099D for deficiency cited. Continue to page 3. Page 3. 1 Type A violation is being cited. The director shall post and provide copies of the report to parents/guardians of the children in care at the facility by the next business day and shall provide to the parents/guardians of children newly enrolled at the facility during the next 12 months. In addition, the director shall immediately post upon receipt the Proof of Correction for 30 consecutive days and provide a copy to current and enrolling parents. The director is to keep Acknowledgement Receipt (LIC 9224) signed by parents in each child’s file . Failure to post Type A reports for 30 days will result in a Civil Penalty of $100.00 Exit interview conducted and report was reviewed with Director Kenya Meneses. The Director was informed that the “Notice of Site Visit” must be posted on or adjacent to the door for 30 days. Failure to post will result in a Civil Penalty of a $100.00. End of Report.
