What the inspector wrote

On 12/04/2025, Licensing Program Analyst (LPA) K. Navar conducted an unannounced Case Management visit to investigate an unusual incident. Upon arrival, the LPA met with facility representative Director Kenya Meneses and informed them of the purpose of the visit. The census at the time of the visit was 44 children and 4 staff and 12 infants and 4 staff. A review of the Facility Personnel Report Summary shows all facility staff or individuals who require caregiver background checks have received a criminal record clearance and a child abuse index clearance or an exemption clearance. The Department received an incident report on 11/19/2025 reported by assistant director Ashley Campos stating that on 11/17/25 that S1 grabbed C1 under the armpits, picked C1 up, and put C1 in the classroom. On 12/04/2025, the LPA interviewed two (2) staff (S1-S2), including one (1) staff (S1) who was supervising children on the day of the incident. S2 stated the children were lining up to go inside the classroom and each of S1’s hands were placed lightly against the back/armpit area of the C1 to guide them into the classroom. S2 stated that the wrong word grabbed was used on the incident report that it was not an aggressive action from S1 but wanted to report it to the childcare licensing. S2 stated they have met with staff during staff development to talk about children’s personal rights. During staff interview with S1, S1 stated that they were transitioning into the classroom and that C1 did not want to go inside so S1 gently placed their hands on the upper body by shoulders and guided C1 in. S1 stated that C1 walked inside the classroom in less than 1 (one) minute. Continue to page 2. Page 2 On 12/04/2025, the LPA obtained the following documents. A copy of staff development training on personal rights of children and a copy of Kiddie Academy Employee Warning Notice. On 12/04/2025 LPA observed where the incident took place on 11/17/2025. During observations S2 described and modeled how the children were lined up, how the teacher placed their hands on child’s back/armpit area, and how C1 entered the classroom. Based on the information gathered from the interviews conducted, observations, and records reviews, it was determined that no deficiency will be given at this time. LPA reviewed and provided a copy of Title 22 Regulations 101212 Reporting Requirements was reviewed with director. An exit interview was conducted with Director Kenya Meneses. The Notice of Site Visit was posted during the visit. The director was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. END.
