What the inspector wrote

On 5/28/2025 at approximately 9:10AM, Licensing Program Analysts (LPAs), Christine Jung and Kathy Trinh, conducted an unannounced Case Management - Deficiency inspection to follow up on an incident that was self-reported to the Department. Upon arrival, LPAs met with Assistant Director. LPAs disclosed the purpose of the inspection and was led on a tour of the facility. Director, Stephanie Stard, arrived at approximately 9:30AM and met with LPAs. There were 27 infant/toddler children, eight (8) infant/toddler staff, 25 preschool children, and three (3) preschool staff members present. Hours of operation are Monday through Friday 6:30AM to 6:30PM. LPAs conducted an inspection on 5/28/2025 regarding an unusual incident report (UIR) submitted to the Department on 5/13/2025 reporting that Child 1 (C1) was fed the wrong bottle. C1 had not finished their bottle, so the bottle was placed on the counter. Staff 1 (S1) attempted to feed C1 again but grabbed Child 2’s (C2) bottle instead. Staff 2 (S2) noticed right away and stopped S1, but C1 had already drank some of the milk from C2’s bottle. Interview was conducted with the Director. Director stated that the facility will implement extra measures to ensure that children’s names are checked and logged before each feeding so that the bottles are not mixed up going forward. The facility reported the incident to the Department in a timely manner. Based on the information obtained and interview conducted, one Type B citation for Infant Care Food Service is being cited on LIC 809D. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Stephanie, Stard.
