Inspector’s narrative

What the inspector wrote

An unannounced case management inspection was conducted on 04/21/2026 by Licensing Program Analyst (LPA), Deschampe arrived at the facility at 9:30AM, in response to a self-reported Unusual Incident Report (UIR) dated 04/16/2026. Present during today’s inspection was the Facility Representative/Director (FR), Barbara Brown. FR guided LPA on tour of the facility with a census of 72 with 18 staff. Total enrollment is 77, with current census due to varied schedules. A review of staff criminal records indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearance or exemption and a child abuse index clearance. On 04/17/2026, the UIR was filed with the Orange County Regional Office (OCRO) to self-report an incident from 04/16/2026 at 5:25PM. Staff 1 (S1) and Staff 2 (S2) were transitioning 5 children from the Toddler room to the Infant room. While transitioning, Child 1 (C1) had walked away from the group. Staff 3 (S3) left the Infant room and found C1 at a buggy with no staff. S3 took C1 back to the Infant room. C1 was away from the group for about 2 minutes. The FR was notified and followed up with staff. The FR informed the parent of the incident. On 04/17/2026, the FR observed and worked with staff during transitions. On this date, LPA made observations, received facility records, attempted to interview C1, and conducted interviews with 9 staff. Due to additional information needed, the UIR needs further investigation . Exit interview conducted and report was reviewed with the facility representative/director, Barbara Brown. Appeal Rights were discussed with FR. The FR was provided a copy of the appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 consecutive days. The “Notice of Site Visit” must be posted on, or immediately adjacent to, the interior side of the main door to the facility. Failure to post will result in Civil Penalties of $100.00. End of Report.
