What the inspector wrote

RP stated on 4/21/25 C1 had fallen and cut their chin open. RP stated C1 was taken to the doctor to get stitched up. RP shared that it was unclear if C1 was playing on the rock structure or the stairs of the structure as there were 2 different stories of what happened from staff. During the course of investigation, LPA toured that facility, reviewed facility\342\200\231s Parent Handbook and reviewed records including medical report. LPA conducted interviews with reporting party, 5 staff members, 4 children, 4 parents and reviewed records. LPA reviewed Unusual Incident Injury Report submitted by facility stating on 4/21/25 at approximately 10:55am C1 and a group of friends were running up the stairs C1 missed a step, fell and bumped their chin on the stair causing a cut and slight bleeding. Regarding allegation (1) Staff left child in soiled clothing while in care LPA interviewed 5 staff members. 5 out of 5 staff members stated they have not witnessed staff leaving a child in soiled clothing while in care. Staff #3 (S3) stated, staff take child to their classroom and get clothes from their cubby, so they can change. Staff put the soil clothes in a bag. Most children can change on their own, and staff will guide them, verbally. Staff will put on gloves and use wipes to clean them. Staff will let the parents know if their child had a bathroom accident, staff tell parents verbally when parents pick up their child. Regarding allegation (2) Child sustained an injury while in care LPA interviewed 5 staff members. 2 out of 5 staff members stated that they had witnessed the incident involving C1 at the facility. Staff #1 (S1) and Staff #2 (S2) stated, C1 was running with their friends and ran up climber steps and fell. S1 applied a paper towel and applied pressure to C1\342\200\231s injury. S1 stated, they informed the Director, and the Director called the parent to inform them of injury. S1 stated, an injury report was completed, and a copy was given to C1\342\200\231s parent/authorized representative. Staff #2 (S2) also stated staff ensure children are being supervised at all times by staff walk around the yard and the classroom, staff make sure all areas are being supervised, staff make sure the kids are following the rules, and make sure the kids are safe. Director stated that staff ensure children are being supervised at all times by staff walk around and keep an eye on all the children, they make sure the children are interacting and playing, and make sure everyone is safe. Director also indicated that staff will keep themselves on opposite sides of the room or playground to make sure the children are being supervised. Page 2 of 3 LPA Duron interviewed 4 children. The 4 children interviewed made no disclosures regarding the above allegations. LPA Duron interviewed 4 parents. All interviewed parents stated they did not have any concern with facility. Based on the information gathered from LPAs\342\200\231 interviews there are insufficient evidence to corroborate the allegations: (1) Staff left child in soiled clothing while in care (2) Child sustained an injury while in care. Although the allegations may have happened or are valid, there are not a preponderance of evidence to prove the allegation did or did not occur in the day care facility, therefore the allegations are UNSUBSTANTIATED. An exit interview was completed. The report was reviewed and discussed. Appeal Rights were provided. The facility representative was provided a copy of their appeal rights and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Licensing office within 15 business days. The facility representative was informed that the \342\200\234Notice of Site Visit\342\200\235 must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00. The \342\200\234Notice of Site Visit\342\200\235 must be posted on or adjacent to the door. Page 2 of 3. End of Report.
