What the inspector wrote

Licensing Program Analyst (LPA) P Rivas conducted an announced pre licensing/plan of correction visit and met with CEO/Director Priya Govindaswamy, and Admin Anand Raghavendran . This facility license is being consolidated into a single license from facility #304371623 During today's visit LPA took census, toured the facility and measured the toddler yard as it is now fenced and set up. LPA toured facility and found, four pre school children and two teachers. The teachers have been fingerprint cleared under 304371623.LPA advised licensee to transfer fingerprint clearances to new license number. The Toddler yard measured at 1569.14 which is sufficient for the capacity requested. The fence is made of plastic material and is at least 4 feet tall and is separate from the pre school yard. Licensee Ms. Priya indicated there has been no changes on the preschool admission agreement and no changes in personnel policies. LPA discussed safe sleep regulations as toddlers ages 18 months to 24 months are considered infants per Community Care Licensing Definitions and infant regulations also must be followed. LPA also viewed that the door leading to the upstairs was locked and inaccessible to children. The licensee has provided an updated lic 200a and lic 999 that reduced toddler capacity request to 12 children and increased the preschool capacity to 36 children. Licensee provided letter regarding the use of the second floor(storage). A new fire clearance with new capacity is still needed. On 11/25/24 a case management inspection was conducted in order to add toddler option component under license #304371623 . The following was noted on the case management visit; "Licensing Program Analyst (LPA) P Rivas conducted an announced case management visit to add a toddler option component ages 18 mos to 36 months in Water room. LPA Met with CEO/Director Priya Page 2 Govindaswamy and Admin Anand Raghavendran and together toured the facility. Applicant is requesting to care for 15 toddler option children ages 18 mo to 36 months and 33 preschool children ages 2 years old to entry into TK or higher grade. Capacity will remain the same, hours of operation will be Monday through Friday 7:30 AM to 6.00 PM Licensees report that there have been no changes to the structures, buildings, fencing, rooms. The facility is a two story building located in a residential area. LPA was advised that the second floor which is made up of two bedrooms, a bathroom and kitchen area will not be used and is off limits. Upon review of second floor the door was unlocked even though there is a bolt lock on door. (the room was blocked by a child's book case)A sketch for second floor had not been provided. Licensee Representatives report that the upstairs is off limits and not for children's use. LPA was advised no one is living in facility and as of now there is no intention of allowing someone to live upstairs. LPA advised if this changes all other laws must be met including notifying Community Care Licensing of changes. A statement was requested. A tour of the facility was conducted, there were 5 staff and a total of 19 children present. Review of Guardian showed staff present had criminal record clearances as required. The Water room which will be for toddler option measured at 435.23 square feet which is insufficient for requested capacity of 15 toddler option children. The Earth and Wind room measured at 1126.13 combined which is sufficient for the requested capacity. Total sinks: 3 X 15= 45 children. Total toilets: 3 X 15=45 children. There is one urinal located in the restroom. One sink and one toilet are designated for the toddler option program. LPA viewed posted sign. Children shall be escorted by a staff person to and from the restrooms. Fire clearance received from Orange County Fire Authority approving the requested capacity of 15 toddler option children and 30 preschool children was received, The total measurements of the preschool rooms do not accommodate the requested capacity for toddler option program. Upon discussion with licensee representatives it was determined that a waiver will not be requested for outdoor activity space but will designate a specific area for the toddler option program. During today's visit LPA did not observe area ready for review. Page 3 Items to be provided 12/02/24. An exit interview was conducted, appeal rights reviewed, notice of site visit provided and must be posted for 30 days. The following is needed prior to approval of toddler option program. 1. The licensees representative will provide a new lic 200a and lic 999 to reduce the toddler option children to 12 and preschool children to 33. The sketch shall include the second floor and the yard shall include the toddler option section. 2. A padlock on door leading to the second floor.(picture can be provided) 3. A separate outside activity space for toddler option program. A visit was scheduled for 12/10/24 to review correction and outside activity space. 4. A letter regarding the use of the second floor. 5. A corrected Fire clearance that matches capacity. LPA advised they must also visit OCFA portal at http://publicservices.ocfa.org/ . Items to be provided 12/02/24. An exit interview was conducted, appeal rights reviewed, notice of site visit provided and must be posted for 30 days." On 12/10/24 a new exit interview was conducted,notice of site visit provided and must be posted for 30 days.
