What the inspector wrote

On 08/06/2025 at 9:05am, Licensing Program Analysts (LPA), Romy Castanon and LPA Silva conducted an announced prelicensing inspection to decrease the facility\342\200\231s preschool capacity and add a toddler option component. This is a single license conversion. The existing license number (304371603) will be consolidated with the new assigned license number (304371743). Upon arrival, LPAs met with Applicant Matt Zaky. Executive Director Amber Zaky was also present during today\342\200\231s visit. Applicant is requesting to be licensed for 24 toddlers ages 18mos-36mos in Room 1 and 2 (previously preschool room), and 53 preschool children ages 2-6 yrs in 3, 4, 5 and 6. Review of Criminal record clearances were verified through Guardian. All staff were fingerprinted and cleared. The hours of operation are Monday-Friday 7:00am to 5:00pm. Room 1 and 2 measured 882 square feet based on prelicensing visit report on 03/02/2023 which is sufficient to meet the capacity requested for the toddler option program. The furniture fixtures and supplies are age appropriate and sufficient for the capacity requested including tables, chairs, cubbies, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via built in filtered water dispenser located in the hallway near the kitchen. Facility provides each child with reusable insulated bottles. LPAs observed all hazardous items to be inaccessible to children. There are 6 toilets and 6 sinks available for children\342\200\231s use and two toilets will be designated for the toddler option children. There are two adult restrooms each equipped with a toilet and sink. A separate staff restroom and front desk area are designated for children who are ill. There are no bodies of water or weapons on the property. The outside activity space measurements based off report dated 03/02/2023 measured 3,290 square feet. The applicant has submitted a waiver to request the playground be shared on a staggered schedule, but additional documentation will be submitted. The equipment on the playground is age appropriate and in good condition. Water is provided via cart with children\342\200\231s water bottles that are refilled from filtered water dispenser. A working carbon monoxide detector was tested in Room 1 and 2 by LPA. Fire clearance was received on 06/05/2025 but needs to be corrected and updated. (Continue to page 2) Page 2 Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. This facility plans to provide Incidental Medical Services \342\200\223 IMS. For IMS information, see PIN 22-02-CCP . A Plan of Operation that includes IMS must be submitted to the Department. LPA reviewed the LIC 311A, Records to Be Maintained at The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted with Applicant Executive Director. LPA discussed the safe sleep regulations with Applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPA also informed Executive Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. (Continue to page 3) Page 3 The following corrections are needed prior to the issuance of the license: 1. Additional photos of playground for toddler component use waiver 2. Waiver request to share restroom with preschool children 3. Corrected and approved fire clearance request 4. Full review of application packet and corrections requested by LPA Applicant understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. A capacity decrease for preschool and toddler component addition may be granted after the above requirements are met and a final file review is conducted. Exit interview conducted and report was reviewed with the Applicant Matt Zaky and Executive Director Amber Zaky. A notice of site visit was given to Applicant and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. End of Report
