What the inspector wrote

Licensing Program Analyst (LPA) Jeremey McClain met with licensing representative Sarah Beeson for an unannounced Case Management Inspection. 1 child was present with 3 staff members. On 10/28/2025, an Unusual Incident Report was emailed to LPA to report that on 10/27/2025, at approximately 9:35 am, Staff 1 observed Child 1 alone in the classroom when they arrived at the facility. It was stated that the child was forgotten during dismissal by Staff 2 and Staff 3 when other children in attendance were dismissed to school. It was stated that Child 1 fell asleep when they arrived around 6:45 am, and reportedly woke up around 8am. Child 1 was not observed to have had any injuries. LPA Jeremey McClain informed facility representative Sarah Beeson that this report dated 10/30/2025 documents a Type A citation which shall be posted for 30 consecutive days as there was an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Jeremey McClain informed facility representative Sarah Beeson to provide a copy of this licensing report dated 10/30/2025 which documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An Immediate Civil Penalty will be assessed for Absence of Supervision. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and this report was reviewed with licensing representative Sarah Beeson. A Notice of Site Visit was provided and shall remain posted for 30 days. Appeal rights were provided.

    Responsibility for Providing Care and Supervision. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by an Unusual Incident Report received on 10/28/2025. Facility self-reported that on 10/27/2025, Child 1 was left alone in the room from approximately 7:40am-9:3ham. This posed an immediate risk to the health, safety, and personal rights of children in care.
