What the inspector wrote

Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Kylie Agnone for an unannounced Case Management Inspection regarding two Unusual Incident Reports. LPAs observed 58 children supervised by 11 staff members. It was reported that on 01/09/2026, Child 1 suffered an injury to their arm after slipping from a bark box and falling on their arm. Child 1 was taken to the hospital after facility called 911 at the parents\342\200\231 request. Child 1 was placed in a cast and is currently prohibited from engaging in physical activity. It was also reported that on 1/21/2026, Child 2 injured their ankle while playing wall ball. It was stated that Child 2 informed staff of their injury, and they were immediately attended to. Child 2 was given a medical boot the following day, which they are no longer wearing. During today\342\200\231s inspection LPA reviewed both children\342\200\231s files, conducted interviews with staff, reviewed doctors\342\200\231 notes and observed the areas where the injuries occurred. No deficiencies were cited as for either incident. An exit interview was conducted, and the report was reviewed with facility representative Kylie Agnone. A Notice of Site Visit was provided and shall remain posted for 30 days.
