Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Christina Ramirez for an unannounced Case Management Inspection regarding an Unusual Incident Report. LPAs observed 38 children supervised by four staff members. It was reported that on 02/06/2026, a child in care suffered a fractured pinky after slipping on a ball on the playground. The child was attended to immediately, however the injury wasn\342\200\231t diagnosed and reported to the facility until 2/12/2026. During today\342\200\231s inspection LPA reviewed C1\342\200\231s file, conducted interviews with C1 and staff and observed the area where the injury occurred. No deficiencies will be cited as a result of the incident. An exit interview was conducted, and the report was reviewed with facility representative Christina Ramirez. A Notice of Site Visit was provided and shall remain posted for 30 days.
