Licensing Program Analyst (LPA) Jeremey McClain met with licensing representative Carla Becerra for an unannounced Case Management Inspection. 1 child was present with 3 staff members. On 01/20/2026, an Unusual Incident Report was emailed to LPA to report that on 01/16/2026, Child 1 and Child 2 had eloped from the facility unsupervised. Staff became aware that Child 1 was not at the facility when they were contacted by the child's guardian. Staff then noticed that Child was also missing. It was stated that Child 1 was last observed walking to the bathroom while other children were gathering at the blacktop. 911 was called. Child 1 was last seen between 2:00-2:15 pm before walking to the bathroom. It was stated that Child 1 and 2 were found at the residence of Child 1 around 3:15 pm. LPA Jeremey McClain informed facility representative Carla Becerra that this report dated 01/29/2026 documents a Type A citation which shall be posted for 30 consecutive days as there was an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Jeremey McClain informed facility representative Carla Becerra to provide a copy of this licensing report dated 01/29/2026 which documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An exit interview was conducted, and this report was reviewed with licensing representative Carla Becerra. A Notice of Site Visit was provided and shall remain posted for 30 days. Appeal rights were provided.
