What the inspector wrote

On Thursday, April 24, 2025, Licensing Program Analyst (LPA) Lea Habtom arrived at Twelve Bridges Elementary School and checked in with the front office. LPA met with site supervisor, Jestine Attaran, to conduct an unannounced case management inspection. The purpose of today's inspection is to add the library as a licensed room. The facility is located on the premises of Twelve Bridges Elementary School and is licensed to use the art room, gym and room #313. The library and room #313 is used by the elementary school during school hours but is vacant once school is dismissed around 2:40 pm. There were no children present during today's inspection. There was 1 staff and the site supervisor present. A health and safety inspection was conducted in the library. LPA did not take measurements on this day as this is a school age program operating on a functioning elementary school site. Staff bring a cart of their supplies to the rooms. Facility uses the school playground and bathrooms. A fire clearance was granted for 250 school age children by Lincoln Fire Department on December 5, 2024. No Title 22 regulations were cited during today's inspection. Exit interview was conducted and a notice of site visit provided to be posted for 30 days. This report was reviewed with site supervisor, Justine Attaran. Effective today, April 24, 2025, the facility is approved to use the library. The facility capacity is 250 with rooms including the art room, gym, room #313 and the library.
