What the inspector wrote

On Friday, October 10, 2025, Licensing Program Analyst (LPA) Lea Habtom arrived to STAR Twelve Bridges which is located on the elementary campus of Twelve Bridges for an unannounced case management inspection. There were no children present at the time of the inspection. Staff were in a in person personal development training in the gym due to school closure. LPA met with Michelle Brashier, Regional Administrator of licensed programs, and disclosed the purpose of today's inspection is to follow up on the unusual incident report received on October 1, 2025. LPA conducted a staff interview and determine the program acted in compliance. Based on the interview, LPA has concluded that there were no Title 22 regulations violated. No Title 22 regulations were cited during today's inspection. This report was reviewed with Michelle Brashier. A notice of site visit was provided to be posted for 30 days. Appeal rights provided.
