What the inspector wrote

Licensing Program Analyst (LPA) Jeremey McClain met with licensing representative Ali Ebert for an unannounced Licensee Initiated Case Management Inspection. Upon arrival LPA observed 75 children supervised by eight staff members. On 07/21/2025, it was reported that on 07/18/2025, a child suffered a fractured arm after an accident on the preschool playground. The incident occurred when the child was playing with another child. Staff 1 heard the child crying and attended to them immediately. The child was found crouching over in the bark area; however, it could not be determined how the child became injured. During today\342\200\231s inspection, LPA interviewed staff present during the incident and reviewed footage of the playground area during the time of the incident. Due to the camera angle the incident was recorded at, LPA was unable to determine how the injury occurred. LPA spoke to the licensing representative about supervision techniques and playground safety. No Title 22 Violations will be issued. An exit interview was conducted, and this report was reviewed with licensing representative Ali Ebert. A Notice of Site Visit was provided and must remain posted for 30 days.
