Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Ali Ebert for an unannounced Case Management Inspection regarding an Unusual Incident Report. LPAs observed 20 napping infants supervised by four staff members. It was reported that on 03/11/2025, a child slipped and fell from a play structure on the playground. The child was attended to right away by the staff member who observed the incident. Although the child didn\342\200\231t show signs of an immediate injury, the child was later seen at the hospital and diagnosed with a broken right clavicle. During today\342\200\231s inspection, LPA conducted a staff interview and observed the play structure and playgroung. No deficiencies were observed during today\342\200\231s inspection. An exit interview was conducted, and the report was reviewed with facility representative Ali Ebert. A Notice of Site Visit was provided and shall remain posted for 30 days.
